A specialty claim is the final expression of several earlier handoffs. The service performed, the documentation in the record, payer requirements and the claim detail need to stay connected from intake through follow-up.
In oncology, for example, scheduling and treatment workflows may involve eligibility, authorization, medication and administration details, documentation and payer review. A pathway view helps teams identify where the required information enters, which handoff owns it and where an exception should be resolved.
The same method adapts to behavioral health, radiology, laboratory, surgery, primary care, DME and other specialties. The checkpoints change with the service. The operating discipline remains consistent: preserve source context, route exceptions to accountable reviewers and use payer outcomes to improve upstream work.
Coding references change over time. CMS publishes ICD-10 files and official guidelines for defined fiscal-year periods. Teams should work from the applicable code set and payer guidance for the date of service rather than treating a static web article as a coding authority.
